Three caregivers are facing felony charges in connection with the death of a resident at a Grand Junction assisted living facility.

Colorado Attorney General Phil Weiser made the announcement Tuesday that charges had been filed against three individuals at Cappella Assisted Living and Memory Care in Grand Junction.

Charges Filed Against Three Caregivers

According to the attorney general's office, an investigation by the Medicaid Fraud Control Unit of the Colorado Department of Law and the Grand Junction Police Department determined that 30-year-old Jamie Johnston, 50-year-old Jenny Logan, and 27-year-old Letticia Martinez were responsible for the death of 86-year-old Hazel Place on June 14.

All three caregivers are charged with negligent death of an at-risk person, a class four felony, and criminally negligent homicide, a class five felony Martinez and Johnston are both charged with second-degree forgery, a class one misdemeanor.

On June 14, Hazel Place, who suffered from Alzheimer's,  reportedly had gone outside in 100-degree temperatures. The attorney general said the investigation revealed  Place was left outside in the heat for six hours unattended - and died.

A Difficult Case For Everyone

Weiser said this is a difficult case for everyone involved.

“When our loved ones are vulnerable and in need of care, Colorado residents should be able to trust their caregivers implicitly.My department is committed to holding accountable those who take advantage of and cause harm to our state’s older residents. Cases of this nature are not only difficult for the family and friends of the victim, but for all those involved. "

According to 9 News, two of the workers charged have been terminated and the third is on leave.

Statement From Cappella

Cappella of Grand Junction, in a statement, said it had launched an internal investigation and had cooperated with the attorney general's investigation. "We are very saddened by the passing of this beloved resident, and we continue to send our sincerest support to this resident's family and friends," the statement said.
