If you live in San Miguel, Ouray or in the Westend of Montrose County you will want to have your voice heard in this health care survey. Tri-County Health Network is a nonprofit organization committed to improving healthcare and services at a lower cost.

One of the problems with healthcare is that everyone’s needs are different and this survey will help assess those needs so a specific community plan can be targeted. The survey will only a few minutes and all of your responses will remain anonymous.
